This paper aimed to assess the implementation process and reach of the extension project UEPG + Ativa – 2nd edition, of the State University of Ponta Grossa, through the analysis of official documents, spreadsheets and reports. It is a cross-sectional, quanti-qualitative study which, from the point of view of program assessment typology, is classified as procedural and prospective, considering that it proposes to analyze whether the target population were being reached, as well as, whether the logical design was being followed. The findings show that the project has been growing positively in terms of its purpose as an extension project. The conclusion reached reveals that this assessment is relevant, seeing that it allows us to present indicators that can demonstrate a view of the current reality to the coordinators, seeking to disclose a collection of information capable of supporting strategies to improve the quality of care and effectiveness of the services offered
