PROPOSTA DE UM INSTRUMENTO DE AVALIAÇÃO DA EXTENSÃO: ESTUDO DE CASO EM UMA INSTITUIÇÃO PÚBLICA FEDERAL BRASILEIRA DE EDUCAÇÃO PROFISSIONAL E TECNOLÓGICA

This article is part of studies on organization, management, and evaluation of Professional and Technological Education. This article aimed to propose a model for an instrument of the extension evaluation with performance indicators that provide consolidated information to the higher spheres of academic governance of the Federal Institute of Education, Science, and Technology of São Paulo. We conducted a qualitative approach research and applied function using the method case study. There is an institutionalization of extension at the Institute in focus, but at the same time, we also identified gaps that concern the evaluation of this academic mission. As result, we proposed 46 indicators for evaluating extension that we organized in two models of forms for collecting information.
